Level 2 Practical Evaluations will continue to be accepted this season.   You may download the paperwork you will need for your Level 2 Practical Evaluations by clicking on the links below. If your word processor is different than WORD, I suggest you save the document in WORD and then try opening it in your own word processing program. That's how I made a Word Perfect Document, downloading it in WORD and then opening and saving it in Word Perfect. I needed to do a little fine tuning in the Player Evaluation Form to make it fit on page by deleting check marks that appeared in every box.

You need to do three Practical Practice Evaluations, each one at least 4 weeks apart. The same evaluator may observe no more than two of the three evaluations. You need to provide each coaching evaluator a copy of the Evaluation Form, and a copy of your Practice Plan. Each evaluator will fill in one of the Evaluation Forms and sign it for you. He/She will also need to sign and make notes on your Practice Plan as well.

Below you will find a link to the ORA Website to obtain printable copies of the Coach Self-Assessment Form and the Coach Player Evaluation Form. You need to fill in the Individual Coaching Record and forward everything to ORA, AFTER all three evaluation are completed. You will see on the Coach Record Form the address to mail it to and a list of what to send. If you are from outside of Ontario, please mail it to your Provincial / Territorial Office which may be found at.
