Complete and submit this form if you would like to officiate in a tournament in the Eastern Region. Assignments are made by a Regional Scheduler. The more specific the information is that you supply, the easier it is to assign the appropriate games.

While Submiting this form,if you get an ERROR Message of any kind, PLEASE Press your Browser BACK BUTTON in the top left corner of your screen to get back to this Page. If your ERROR Message says something to the effect of: "Sorry, but I cannot figure out who sent you here. Your browser is not sending an HTTP_REFERER. If you are using Norton Firewall (any version), please see the Norton support site." The Norton site will NOT help. I got that ERROR Message when testing the system and found that out after trying for hours to find out what the message meant. I eventually found that clicking on my Norton Internet Security Icon in the Lower Right hand corner of my screen, I was able to select "disable" which then allowed me to click Submit with no error message. As soon as I submitted the form, I immediately selected "enable" my Norton Security System again. My Internet Security was off for approximately 3 seconds..
